Would it be wise to get my diamond appraised? Is there any other way to find out whether the diamond is real or not?

Unless you are an experienced gemologist with a laboratory at your disposal, you have no way of being sure that the diamond on your engagement ring is real. The only way you can be 100% sure that your diamond is a real diamond is by bringing it to a diamond grading lab or an accredited gemologist. You should already have a diamond certificate with your diamond, but old vintage diamonds aren't always certified, or if they are they may not be up to today's standards. I recommend you check online for the nearest diamond grading lab, or send it to GIA and have them issue a diamond grading report. It's worth spending a few more bucks in exchange for peace of mind.

In olden days people used to check the authenticity of diamonds by scratching them against a hard surface, a mirror for instance. If the diamond scratched the mirror, it was real, if however the diamond was scratched, then it was fake. Presumably, diamonds are the hardest substance in nature, so many people believe that a real diamond would be indestructible. In truth, a diamond is incredibly durable, however it is also very brittle since it is made out of almost 100% pure carbon. Therefore, a diamond could be shattered with a swift blow from a blunt object such as a hammer. Nowadays, diamonds are certified by the GIA or AGS as real. If your diamond does not have a certification and you are in doubt as to its authenticity, then simply take it to a GIA approved gemologist and have it appraised. In addition, if the diamond is real I would recommend getting the precious gemstone insured against any possible damage or loss.
